Complete the Membership Application
form and supporting documents (a list of
required supporting documents can be
found on page 4 of the application form),
then email all documents to info@massagemyotherapy.com.au. |
When the Association has reviewed your application, you will be asked to make payment. |
You are now a member and the process of forwarding your details to the private health funds will begin. |
Joining from Another Association?
